Defect

The slipper pads in certain electric models and the brake pads in certain nitro fuel models may contain chrysotile asbestos, which may be released into the air through friction or if the clutch assembly or brake pads are disassembled.

Hazard

There is a risk of serious medical conditions, including asbestosis, lung cancer and mesothelioma if asbestos fibres are released into the air and breathed in.

What to do

Consumers should stop using affected vehicles immediately and must not attempt to remove or replace the slipper or brake pads, or disassemble the clutch assembly.

Affected vehicles should be returned to their place of purchase to be sent to a centralised repair facility for a free repair.

If your vehicle was bought second-hand, your place of purchase is no longer in business or you do not remember where you purchased the vehicle, you should contact Southern Model Supplies for further instructions.

Defect

The x-capacitors in the power board can deteriorate and may fail unsafely at the rear of the tv which can cause a flame and emit smoke.

Hazard

Risk of serious injury or electric shock and risk of smoke inhalation or flame to occur. In circumstances where there is a flame or smoke being emitted, do not use any liquid to extinguish as this poses the risk of electric shock and serious injury to occur. Immediately leave the vicinity of the tv and turn the main power to the property off.

What to do

Consumers with affected TVs should contact LG Electronics Australia on 1800 643 156 to arrange a technician to replace the power board free of charge or in the case of irreparable damage, replacement of the tv free of charge.

The raindrop cloud toy includes small raindrops that may detach from the toy. Incidents have occurred where the toy’s raindrop has detached and resulted in children putting them in their mouths.

Hazard

A detached raindrop may pose a choking or suffocation hazard for infants and young children, which can lead to severe injury or death.

What to do

When not in use, and an infant is not secured in the swing or rocker, the restraint straps may dangle below the seat. This may lead to children becoming tangled in the strap.

Defect

The rotating swing handle on the cookware has a pivot-lock latching system to lock the handle in place for a secure grip when in use.

If the user fails to properly latch and lock the handle or if the latch releases, the pot could become unstable with a left-handed pour.

The product in its 12 month age mode has key features of a stroller, but does not have the safety features required by the mandatory safety standard for strollers.

Hazard

When used as a stroller to transport children in its 12 month age mode, the product may cause injury to a child if the product rolls in an uncontrolled manner or if a child falls out of the product.

What to do

Consumers should not use the product as a stroller to transport children. Consumers should remove and discard the high headrest and shoulder straps on the tricycle and follow updated Eurotrike guidance that children younger than 18 months of age not use the modified product. What to do

Customers should stop using affected adapters and exchange them at no charge for a new redesigned model that is currently shipping. Customers should visit http://www.apple.com/au/support/ac-wallplug-adapter/ for information about how to identify the affected adapters and the exchange process. Customers can fill out a web form with their shipping address, email and product serial number and Apple will send the new adapter to the customer. The package will also contain a postage pre-paid package, which the customer can use to return the affected adapter to Apple.

Customers can also visit an Apple Retail or an Apple Authorised Service Provider location to exchange their affected adapters.
